Наиболее распространённым методом диагностики сердечно-сосудистой токсичности является оценка структуры и функции левого желудочка.</p><sec><title>Цель</title><p>Цель: исследование структурно-функциональных изменений миокарда левого желудочка и левого предсердия у пациентов с острым миелоидным лейкозом в процессе химиотерапевтического лечения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включено 110 пациентов с впервые выявленным острым миелоидным лейкозом, которым показано проведение полихимиотерапии с применением антрациклиновых препаратов. Проводилась оценка показателей структуры и функции левого желудочка на основании трансторакальной эхокардиографии с применением методики specletracking и определением глобальной продольной деформации левого желудочка.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с острым миелоидным лейкозом и верифицированной кардиоваскулярной токсичностью выявлялось достоверное изменение размеров (КДР, КСР) и объёма левого желудочка (КДО, КСО). Нарушение систолической функции проявлялось снижением фракции выброса левого желудочка и относительной глобальной продольной деформации, при этом уменьшение GLS ЛЖ предшествовало снижению ФВ ЛЖ. Диастолическая дисфункция у пациентов с выявленным сердечно-сосудистыми осложнениями антрациклин-содержащей полихимиотерапии проявлялась снижением соотношения параметров Е/А, e’септальный, е’латеральный.</p></sec><sec><title>Выводы</title><p>Выводы. У пациентов с острыми миелоидными лейкозами, получающих антрациклин-содержащую полихимиотерапию, выявляются структурно-функциональные изменения миокарда левого желудочка, заключающиеся в увеличении размера и объёма левого желудочка, а также в возникновении систолической и диастолической дисфункции. Достоверным признаком кардиоваскулярной токсичности является снижение глобальной продольной деформации левого желудочка. </p></sec></abstract><trans-abstract xml:lang="en"><p>Cardiovascular toxicity is one of the most common complications of chemotherapeutic treatment. The most common method for diagnosing cardiovascular toxicity is to assess the structure and function of the left ventricle.</p><p>The aim of the study was to investigate structural and functional changes in the myocardium of the left ventricle and left atrium in patients with acute myeloid leukemia during chemotherapeutic treatment.</p><sec><title>Materials and methods</title><p>Materials and methods: the study included 110 patients with newly diagnosed acute myeloid leukemia who were prescribed polychemotherapy with anthracycline drugs. The parameters of the structure and function of the left ventricle were assessed based on transthoracic echocardiography using the specle-tracking technique and determination of the global longitudinal deformation of the left ventricle. </p></sec><sec><title>Results</title><p>Results. In patients with acute myeloid leukemia and verified cardiovascular toxicity, significant changes in the size (EDR, SSR) and volume of the left ventricle (EDV, ESV) were detected. Impaired systolic function was manifested by a decrease in the left ventricular ejection fraction and relative global longitudinal deformation, with a decrease in LV GLS preceding a decrease in LV EF. Diastolic dysfunction in patients with identified cardiovascular complications of anthracycline-containing polychemotherapy was manifested by a decrease in the E/A ratio, e' septal, e' lateral.</p></sec><sec><title>Conclusions</title><p>Conclusions: In patients with acute myeloid leukemia receiving anthracycline-containing polychemotherapy, structural and functional changes in the left ventricular myocardium are detected, consisting in an increase in the size and volume of the left ventricle, as well as in the occurrence of systolic and diastolic dysfunction.
