Брюшина является активной серозной оболочкой, участвующей в обеспечении барьерной, транспортной, иммунной и репаративной функций. При воспалении ее морфофункциональные свойства существенно изменяются, что определяет течение перитонита и развитие осложнений.Цель. Обобщить современные данные об анатомии, физиологии брюшины и особенностях ее реакции на воспаление.Материал и методы. Проведен анализ отечественных и зарубежных литературных источников, посвященных гистологии, микроциркуляции, лимфодренажу, мезотелиальной функции и патофизиологии воспаления брюшины.Результаты. Показано, что брюшина представляет собой высокоспециализированный орган с выраженной структурной и функциональной неоднородностью. Мезотелиальные клетки, гликокаликс, субмезотелиальная строма, сосудистая и лимфатическая сети обеспечивают ее барьерные и фибринолитические свойства. При воспалении активируются экссудация, миграция лейкоцитов, экспрессия молекул адгезии и цитокинов, нарушается баланс между фибриногенезом и фибринолизом, что способствует формированию фибрина, спаек и фиброза.Заключение. Брюшина должна рассматриваться как динамичный иммунно-метаболический орган. Изучение ее реакции на воспаление имеет важное значение для понимания патогенеза перитонита и совершенствования его лечения.</p></abstract><trans-abstract xml:lang="en"><p>Background. The peritoneum is an active serous membrane involved in barrier, transport, immune, and reparative functions. Inflammation significantly alters its morphofunctional properties, thereby influencing the course of peritonitis and the development of complications.Objective. To summarize current knowledge on the anatomy and physiology of the peritoneum and its response to inflammation.Materials and methods. A review of Russian and international literature was performed, focusing on peritoneal histology, microcirculation, lymphatic drainage, mesothelial function, and the pathophysiology of peritoneal inflammation.Results. The peritoneum was shown to be a highly specialized organ with marked structural and functional heterogeneity. Mesothelial cells, the glycocalyx, submesothelial stroma, vascular and lymphatic networks provide barrier and fibrinolytic properties. During inflammation, exudation, leukocyte migration, expression of adhesion molecules and cytokines are activated, while the balance between fibrin formation and fibrinolysis is disrupted, promoting fibrin deposition, adhesions, and fibrosis.Conclusion. The peritoneum should be regarded as a dynamic immunometabolic organ.
