Хроническая инсомния является одним из наиболее распространённых нарушений сна, встречаясь у 10–15% взрослого населения. Это состояние сопровождается снижением качества жизни, когнитивными расстройствами и повышением риска соматических заболеваний. Современные методы функциональной магнитно-резонансной томографии (МРТ) позволяют исследовать особенности нейронной активности мозга в состоянии покоя и оценивать взаимодействие функциональных сетей, в том числе сенсомоторной (SMN), визуальной (VN) и фронтопариетальной (FPN). По данным зарубежных исследований, при хронической инсомнии отмечаются изменения внутрисетевой и межсетевой коннективности, включая ослабление интеграции фронтопариетальной сети. В отечественных работах подобные данные немногочисленны, а взаимосвязь показателей функциональной коннективности с объективными характеристиками сна по данным полисомнографии (ПСГ) изучена недостаточно.Цель исследования: выделить основные функциональные сети покоя у пациентов с хронической инсомнией и оценить взаимосвязи между сетевой коннективностью и объективными параметрами сна.Материалы и методы. В исследование включён 51 пациент с клинически подтверждённой хронической инсомнией и 34 здоровых добровольца, сопоставимых по полу и возрасту. Все пациенты обследованы на базе ФГБУ «НМИЦ им. В.А. Алмазова» Минздрава России. Полисомнография выполнялась на системе Embla N7000 (Natus, США) с использованием программы Somnologica Studio по стандартному протоколу. МРТ проводилась на томографе Magnetom Trio A Tim (Siemens) с индукцией магнитного поля 3,0 Тесла. Протокол включал структурную последовательность (T1-MPRAGE) и функциональную (EPI BOLD, resting-state). Предобработка данных включала коррекцию движения, временную синхронизацию, нормализацию и сглаживание. Независимый компонентный анализ (ICA) выполнялся с использованием пакетов SPM12, CONN Toolbox и DPABI. Статистическая обработка проводилась с применением двухвыборочных t-тестов и поправкой на множественные сравнения (FDR, p&lt;0,05).Результаты и обсуждение. По данным независимого компонентного анализа у пациентов с хронической инсомнией выявлены изменения функциональной организации сенсомоторной, визуальной и фронтопариетальной сетей. У лиц с нарушением структуры сна наблюдалось ослабление связности сенсомоторной сети с височными ассоциативными зонами, что отражает неполную деактивацию сенсорных контуров при засыпании. Ослабление взаимодействия визуальной сети с мозжечковыми и теменно-височными областями указывает на нарушение сенсорного торможения и избыточную активность зрительных контуров. У пациентов с ухудшением качества сна и увеличением латентности засыпания отмечено ослабление связности фронтопариетальной сети с лобными и теменно-височными областями, что свидетельствует о дисфункции механизмов когнитивного торможения. Полученные данные подтверждают концепцию гипервозбуждения при хронической инсомнии, характеризующуюся сохранением активности исполнительных и сенсорных систем даже в состоянии покоя.Заключение. У пациентов с хронической инсомнией выявлены изменения функциональной организации сенсомоторной, визуальной и фронтопариетальной сетей, проявляющиеся снижением их связности с ассоциативными и исполнительными областями мозга. Эти изменения отражают нарушение процессов сенсорной и когнитивной деактивации, необходимых для перехода ко сну, и соответствуют концепции гипервозбуждения при инсомнии. Полученные результаты позволяют уточнять нейрофизиологические механизмы расстройств сна и могут быть использованы для поиска объективных МРТ-маркеров нарушений регуляции цикла сон–бодрствование.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Chronic insomnia is a common sleep disorder, affecting approximately 10–15% of adults. It is associated with reduced quality of life, cognitive disturbances, and decreased risk of somatic diseases. Functional magnetic resonance imaging (fMRI) allows to assess resting-state neural activity and analyze major brain networks interactions, including sensorimotor network (SMN), visual network (VN), and frontoparietal network (FPN). Previous international studies demonstrated that chronic insomnia involves alterations in both intra- and inter-network connectivity, characterized by increased default mode network activity and reduced frontoparietal integration. However, similar findings in Russian populations are scarce, and associations between functional connectivity measures and objective polysomnographic (PSG) sleep parameters remain insufficiently investigated.The study aimed to identify major resting-state functional networks in patients with chronic insomnia, assess internetwork connectivity and its correlation with objective sleep parameters.Materials and methods. The study included 51 patients with clinically confirmed chronic insomnia and 34 healthy volunteers. All participants underwent examination at the Almazov National Medical Research Centre, Ministry of Health of the Russian Federation. Polysomnography was performed using the Embla N7000 system (Natus, USA) and Somnologica Studio software in accordance with the standard protocol. Magnetic resonance imaging was conducted using Magnetom Trio A Tim scanner (Siemens, Germany) with a magnetic field strength of 3.0 T. The protocol comprised a structural T1-MPRAGE sequence and a functional EPI-BOLD sequence (resting state). Data preprocessing included motion correction, slice-timing adjustment, normalization, and spatial smoothing. Independent component analysis (ICA) was performed using SPM12, CONN Toolbox, and DPABI. Statistical analysis was conducted using two-sample t-tests with false discovery rate (FDR) correction for multiple comparisons (p&lt;0.05).Discussion. Independent component analysis revealed altered functional organization of the sensorimotor, visual, and frontoparietal networks in patients with chronic insomnia. In individuals with disrupted sleep architecture, reduced connectivity between the sensorimotor network and temporal associative regions indicated incomplete deactivation of sensory circuits during the transition to sleep. Decreased connectivity of the visual network with cerebellar and parietotemporal regions suggested impaired sensory inhibition and persistent activation of visual pathways. In patients with reduced sleep quality and prolonged sleep latency, weakened connectivity of the frontoparietal network with frontal and parietotemporal regions reflected impaired cognitive inhibition. Obtained data support the hyperarousal model of chronic insomnia, characterized by sustained activation of executive and sensory networks even during rest.Conclusion. Patients with chronic insomnia showed altered functional organization of the sensorimotor, visual, and frontoparietal networks, characterized by reduced connectivity with associative and executive brain regions. These changes indicate impaired sensory and cognitive deactivation processes necessary for the transition to sleep and align with the hyperarousal model of insomnia.
