Телемедицинские консультации (ТМК) в настоящее время являются неотъемлемой частью работы системы здравоохранения Российской Федерации. Преодолевая и сокращая такие известные факторы, как время и расстояние, ТМК тем самым обеспечивают повышение доступности медицинской помощи без потери её качества. Имплементация телемедицинского модуля в торакальной хирургии во многих странах характеризуется недостаточностью использования этого ресурса, несмотря на очевидные доказанные преимущества технологии.</p></sec><sec><title>Цель</title><p>Цель: провести анализ годовых результатов использования ТМК при оказании медицинской помощи пациентам торакального хирургического профиля в Самарской области.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В основу данного анализа положен опыт работы имплементации и проведения ТМК в хирургическом торакальном отделении ГБУЗ «Самарская областная клиническая больница им. В.Д. Середавина» в период с 1 января 2025 г. по 31 декабря 2025 г. В исследовании были применены следующие методы – описательный, статистический, социологический. Сравнительная часть построена на материалах баз данных PubMed, Medline, ELibrary. Обработка статистических данных проведена с помощью валидированного статистического пакета SPSS. При анализе результатов эффективности ТМК оценивали не только непосредственные лечебно-диагностические результаты и итоги пациентского сопровождения, но и качество технологических и логистических процессов при организации телекоммуникации. За указанный период было проведено 138 ТМК в формате «Врач–Врач» и 113 ТМК в формате «Врач–Пациент».</p></sec><sec><title>Результаты</title><p>Результаты. При разумном системном подходе к внедрению телемедицинских консультаций их эффективность во многих случаях не уступает эффективности аналогичных традиционных услуг при сравнимой безопасности и значительной экономии ресурсов как пациента, так и системы здравоохранения. Широкое внедрение ТМК в практику торакальной хирургической службы способствовало оптимизации графиков работы специалистов. Количество выездов  в ЛПУ – заявителя ТМК сократилось на 28,6% – с 133 в 2023 г. до 95 в 2025 г. (р˂0,01). Также на 15,6% уменьшилось количество повторных запросов на консультацию, что свидетельствует о полноте и качестве получаемых заключений и рекомендаций (р˂0,01). В то же время проведение ТМК позволяет оценить качество предоставляемой медицинской помощи в ЛПУ первичной госпитализации.</p></sec><sec><title>Заключение</title><p>Заключение. ТМК в организации деятельности торакальной хирургической помощи Самарской области в настоящее время находятся на стадии устойчивого развития. Обеспечение высокого уровня доступности данного специализированного раздела достигается, в том числе, за счёт усовершенствования базовых модулей и повышения приверженности медицинского сообщества. Результаты лечения пациентов с применением ТМК делают ощутимый вклад в снижение смертности населения от управляемых причин. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Telemedicine consultations (TMC) are currently an integral part of the Russian Federation's healthcare system. By overcoming and reducing such well-known factors as time and distance, TMC thereby increase the availability of medical care without losing its quality. The implementation of the telemedicine module in thoracic surgery in many countries is characterized by insufficient use of this resource, despite the obvious proven advantages of the technology.</p></sec><sec><title>Aim</title><p>Aim: to analyze the annual results of using TMC in providing medical care to thoracic surgical patients in the Samara Region.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. This analysis is based on the experience of implementing and conducting TMK in the surgical thoracic department of the Samara Regional Clinical Hospital named after V.D. Seredavin from January 1, 2025, to December 31, 2025. The following methods were used in the study: descriptive, statistical, and sociological. The comparative part is based on data from the PubMed, Medline, and ELibrary databases. Statistical data was processed using the validated statistical package SPSS. When analyzing the effectiveness of TMC, we evaluated not only the immediate medical and diagnostic results and the outcomes of patient support, but also the quality of technological and logistical processes in the organization of telecommunications. During this period, 138 TMC were conducted in the "Doctor–Doctor" module and 113 TMC were conducted in the "Doctor–Patient" module.</p></sec><sec><title>Results</title><p> Results. With a reasonable systematic approach to the implementation of telemedicine consultations, their effectiveness in many cases is not inferior to that of similar traditional services with comparable safety and significant resource savings for both the patient and the healthcare system. The widespread introduction of TMС into the practice of thoracic surgical services has helped optimize the work schedules of specialists. The number of visits to TMС applicant health facilities decreased by 28.6% from 133 in 2023 to 95 in 2025 (p ˂ 0,01). The number of repeated consultation requests also decreased by 15.6%, which indicates the completeness and quality of the conclusions and recommendations received (p ˂ 0,01). At the same time, conducting TMС allows for assessing the quality of medical care provided in the primary hospitalization facility.</p></sec><sec><title>Conclusion</title><p>Conclusion. TMK in the organization of thoracic surgical care in the Samara region is currently at the stage of sustainable development. Ensuring a high level of accessibility to this specialized section is achieved, among other things, by improving the basic modules and increasing the commitment of the medical community.
