Ишемически-реперфузионное повреждение (ИРП) остаётся значимым осложнением после трансплантации печени, ассоциированным с ранней дисфункцией трансплантата и худшими исходами. Прогнозирование риска ИРП могло бы направлять выбор стратегий консервации органов и посттрансплантационного ведения.</p></sec><sec><title>Цель</title><p>Цель: разработать и валидировать клиническую систему оценки риска для прогнозирования тяжёлого ИРП при трансплантации печени.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ретроспективный анализ 1238 трансплантаций печени, выполненных в НИИ скорой помощи им. Н.В. Склифосовского с 2000 по 2025 год. Тяжёлое ИРП определялось как пиковые значения трансаминаз &gt;1000 МЕ/л в первые 7 суток. Оценивались донорские, процедурные и реципиентные факторы с использованием многофакторной логистической регрессии для создания упрощенной системы оценки риска.</p></sec><sec><title>Результаты</title><p>Результаты. Полные данные были доступны для 827 пациентов, среди которых зарегистрировано 312 случаев тяжёлого ИРП, что составило 37,7% когорты. Независимыми предикторами явились возраст донора с отношением шансов 1,04 на каждый год, время холодовой ишемии с отношением шансов 1,17 на каждый час и тромбоз воротной вены с отношением шансов 1,52. Разработанный Индекс Риска ИРП (IRI Score) в диапазоне от 0 до 9 баллов стратифицировал пациентов на группы низкого риска при 0–2 баллах с частотой ИРП 35,6%, промежуточного риска при 3–5 баллах с частотой 42,1% и высокого риска при 6 и более баллах с частотой 52,2%. Площадь под ROC-кривой составила 0,555 при p&lt;0,001. Пациенты высокого риска составили 2,8% когорты.</p></sec><sec><title>Заключение</title><p>Заключение. IRI Score обеспечивает раннюю идентификацию пациентов с повышенным риском тяжёлого ИРП и предоставляет конкретный алгоритм для принятия решений о применении машинной перфузии. Простота индекса и использование предтрансплантационных переменных делают его практичным для клинического применения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Ischemia-reperfusion injury (IRI) remains a significant complication following liver transplantation, associated with early allograft dysfunction and worse outcomes. Predicting the risk of IRI could guide the selection of organ preservation strategies and posttransplant management.</p></sec><sec><title>Objective</title><p>Objective. To develop and validate a clinical risk scoring system for predicting severe IRI in liver transplantation.</p></sec><sec><title>Materials and Methods</title><p> Materials and Methods. A retrospective analysis of 1,238 liver transplantations performed at the N.V. Sklifosovsky Research Institute for Emergency Medicine from 2000 to 2025 was conducted. Severe IRI was defined as peak transaminase levels &gt;1,000 IU/L within the first 7 postoperative days. Donor, procedural, and recipient factors were evaluated using multivariable logistic regression to construct a simplified risk scoring system.</p></sec><sec><title>Results</title><p>Results. Complete data were available for 827 patients, among whom 312 cases of severe IRI were recorded, accounting for 37.7% of the cohort. Independent predictors included donor age with an odds ratio of 1.04 per year, cold ischemia time with an odds ratio of 1.17 per hour, and portal vein thrombosis with an odds ratio of 1.52. The developed IRI Risk Index (IRI Score), ranging from 0 to 9 points, stratified patients into low-risk (0–2 points) with an IRI rate of 35.6%, intermediate-risk (3–5 points) with a rate of 42.1%, and high-risk (≥6 points) with a rate of 52.2%. The area under the ROC curve was 0.555 (p&lt;0.001). High-risk patients comprised 2.8% of the cohort.</p></sec><sec><title>Conclusion</title><p>Conclusion. The IRI Score enables early identification of patients at elevated risk for severe IRI and provides a specific algorithm for decision-making regarding the use of machine perfusion.
