Проведён анализ 25 публикаций на английском языке, найденных в базах данных PubMed, MEDLINE и Google Scholar. Исследование охватывало вопросы хирургической тактики, техники формирования кишечных стом, техники пластики грыж, а также ближайших и отдалённых результатов лечения и их осложнений.Результаты. Частота сочетания послеоперационных вентральных грыж с кишечными стомами колеблется от 6,5% до 14,8%. Число кишечных стом неуклонно растёт вследствие увеличения объёма экстренных операций при осложнённых заболеваниях органов брюшной полости. Парастомальная грыжа является наиболее частым осложнением коло- и энтеростомий; через пять лет после её устранения риск повторного вмешательства достигает 19%. Ключевыми факторами профилактики грыж признаются предоперационная разметка места выведения стомы, выбор латерального доступа через прямую мышцу живота и профилактическая установка сетчатого импланта. Вопрос о допустимости одномоментного закрытия кишечной стомы и пластики брюшной стенки остаётся дискуссионным: ряд авторов подтверждает безопасность комбинированного вмешательства и преимущества для качества жизни пациента, тогда как другие настаивают на поэтапном подходе, особенно при наличии хронической инфекции. Применение воронкообразных интраперитонеальных сеток позволяет снизить частоту рецидивов парастомальных грыж до 4,8%.Заключение. Единый стандарт хирургического лечения больных с кишечными стомами и послеоперационными вентральными грыжами не разработан. Необходимы масштабные проспективные исследования, которые позволят определить оптимальную тактику и этапность хирургических вмешательств в данной группе пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Objective: To present a review of foreign literature devoted to the treatment of patients with intestinal stomas and postoperative ventral hernias.Materials and methods. Analysis of 25 English-language publications identified in PubMed, MEDLINE, and Google Scholar databases. The study covered surgical tactics, techniques of intestinal stoma formation, hernia repair techniques, as well as immediate and long-term treatment outcomes and their complications.Results. The incidence of postoperative ventral hernias combined with intestinal stomas ranges from 6.5% to 14.8%. The number of intestinal stomas is steadily increasing due to increased volume of emergency operations for complicated abdominal organ diseases. Parastomal hernia is the most common complication of colostomy and enterostomy; five years after its elimination, the risk of repeat intervention reaches 19%. Key factors for hernia prevention include preoperative stoma site marking, selection of a lateral approach through the rectus abdominis muscle, and prophylactic mesh implant placement. The question of the permissibility of simultaneous closure of intestinal stoma and abdominal wall repair remains controversial: some authors confirm the safety of combined intervention and its advantages for patient quality of life, while others insist on a staged approach, especially in the presence of chronic infection. The use of funnel-shaped intraperitoneal meshes reduces the recurrence rate of parastomal hernias to 4.8%.Conclusion. A uniform standard for surgical treatment of patients with intestinal stomas and postoperative ventral hernias has not been established.
