Переломы средней и верхней трети малоберцовой кости с одновременным разрывом дистального межберцового синдесмоза (ДМБС) являются достаточно редким видом травмы, характеризующиеся техническими трудностями репозиции и фиксации, и, зачастую, неблагоприятным прогнозом.</p></sec><sec><title>Цель</title><p>Цель: проанализировать источники литературы, посвящённые проблеме фиксации ДМБС в условиях перелома верхней трети малоберцовой кости. Представить серию из трёх клинических случаев лечения данной патологии при помощи гибридной техники – одновременного использования ригидной и гибкой систем фиксации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Нами были проанализированы источники литературы, относящиеся к теме фиксации разрывов дистального межберцового синдесмоза в условиях высоких переломов малоберцовой кости в базах данных PubMed, Embase, Google Scholar, The Cochrane Database of Systematic Reviews КиберЛенинка по запросам: межберцов*, позиционный винт, пуговичный фиксатор, перелом заднего края, надсиндесмозный перелом малоберцовой кости. Также нами был применён метод одновременного использование позиционного винта и пуговчатого фиксатора при разрывах ДМБС с одновременным высоким переломом малоберцовой кости у трёх пациентов и проанализированы отдалённые результаты лечения.</p></sec><sec><title>Результаты</title><p>Результаты. Анализ литературы показал, что ряд исследований выявил недостаточную степень стабилизации малоберцовой кости в кранио-каудальном и сагиттальном направлении при использовании пуговчатых фиксаторов. При этом пуговчатые фиксаторы обеспечивают максимальную стабильность от латерального смещения. Одновременное использование пуговчатого фиксатора и позиционного винта позволило в нашей серии наблюдений получить отличные результаты лечения у пациентов с разрывом ДМБС и высоким переломом малоберцовой кости. Обсуждение. До настоящего момента не существует единой точки зрения касательно оптимального метода фиксации разрывов ДМБС с одновременным высоким переломом малоберцовой кости. Данный тип повреждений сопровождается многовекторной нестабильностью малоберцовой кости в вырезке большеберцовой кости. Изолированное использование пуговчатого фиксатора или позиционного винта имеет ряд ограничений.</p></sec><sec><title>Заключение</title><p>Заключение. Одновременное использование позиционного винта и пуговчатого фиксатора может улучшить результаты лечения данной категории пациентов, ускорить реабилитацию и обеспечить более полное восстановление функции, снизить необходимость в повторных операциях.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Fractures of middle and upper part of the fibula combined with syndesmosis rupture is a subtype of malleolar fracture, characterized by technical difficulties in reposition and fixation, and often bad prognosis. Goal.</p><p>The goal of our research was to make an analysis of the literature, related to the problem of fixation of syndesmosis rupture in conjunction with fractures of middle and upper part of the fibula, and to represent case series of such patients treated with our method - combined syndesmosis screw and suture-button.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comprehensive literature search was conducted in the electronic databases PubMed, Embase, Google Scholar, The Cochrane Database of Systematic Reviews CyberLeninka. Also we treat 3 patients with our method – combined syndesmosis screw and suture-button, and analyse short-term, middle-term and long term results of such treatment strategy. Results. Analysis of literature revealed that isolated use of suture button in situation of fixation of syndesmosis rupture in conjunction with fractures of middle and upper part of the fibula, do not provide stabilization of fibula in cranio-caudal and saggital plane. In such cases combined syndesmosis screw and suture-button usage could be beneficial. In our small case series we get good to excelent results.</p></sec><sec><title>Discussion</title><p>Discussion. Until now there is no consensus regarding best treatment option for syndesmosis rupture in conjunction with fractures of middle and upper part of the fibula. This type of fracture is accompanied by multi-directional instability of the distal fibula. Isolated usage of syndesmosis screw and suture-button has a number of restrictions.</p></sec><sec><title>Conclusion</title><p>Conclusion.
