Радиомика – стремительно развивающееся направление в сфере онкологической визуализации, благодаря которому становится возможным поиск признаков, прогностически эффективных в отношении конкретных генетических событий, обусловливающих различные характеристики течения заболевания. Согласно данным многочисленных исследований, наличие IDH-мутации при глиальных опухолях ассоциировано с большей общей выживаемостью. Биопсия, являющаяся «золотым стандартом» дифференциальной диагностики опухолей головного мозга, зачастую затруднительна в связи со сложностью оперативного доступа, периодической невозможностью повторной процедуры, серьёзными осложнениями и летальностью.</p></sec><sec><title>Цель</title><p>Цель: поиск визуализационных признаков, предоставляющих прогностические данные о наличии мутаций и уровнях экспрессии молекулярно-генетических маркеров глиом, получаемых с использованием традиционных импульсных МР-последовательностей и характеризующихся отсутствием ограничений по применимости в зависимости от видимых морфологических особенностей новообразований.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: ретроспективный анализ первичных данных магнитно-резонансной томографии (МРТ) головного мозга 49 пациентов, соответствующих критериям включения в исследование, с глиальными опухолями и впоследствии идентифицированным статусом целевой иммуногистохимической переменной за 2021–2023 гг. из архива ФГБУ «НМИЦ им. В.А. Алмазова» МЗ РФ (n = 31) и ГБУЗ «СПб КНпЦСВМП(о) им. Н.П. Напалкова» (n = 18); препроцессинг исходных МР-изображений по методике сопоставления гистограмм; определение и посрезовая полуавтоматизированная сегментация зон интереса с последующей экстракцией присущих им радиомических характеристик; поиск радиомических признаков, обладающих прогностической способностью в отношении целевой переменной, с использованием инструментов предиктивного статистического анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена высокая эффективность радиомического признака Dependence Entropy в качестве предиктора IDH-мутаций (площадь под ROC-кривой – 0,766 [0,627–0,880]).</p></sec><sec><title>Выводы</title><p>Выводы. Обоснованно определены: целевая иммуногистохимическая характеристика для разработки предиктивной модели (IDH-статус), рабочая импульсная последовательность (Т2-Tirm), инструмент препроцессинга исходных данных визуализации (методика сопоставления гистограмм), зоны интереса (участки гиперинтенсивности на Т2-Tirm, ассоциирующиеся с опухолевым поражением, с захватом связанных кистозных и/или некротических компонентов). В результате выявлена статистически достоверная взаимосвязь между радиомической характеристикой Dependence Entropy и IDH-статусом глиальных опухолей. В ходе дальнейшей работы планируется увеличение базы данных, повышение точности имеющейся статистической модели, поиск релевантных радиомических характеристик, экстрагируемых с использованием иных традиционных импульсных последовательностей, создание комплексной предиктивной радиогеномной модели, разработка автоматизированного программного обеспечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Radiomics is a rapidly developing field in oncology visualization aimed at searching for prognostically effective imaging features associated with specific genetic events that determine various characteristics of the disease course. According to numerous studies, the presence of IDH mutations in glial tumors determines a longer overall survival. Despite the fact that biopsy is considered to be the «gold standard» for brain tumors differential diagnosis, it is though quite difficult to perform due to the complexity of surgical access, common cases of the repeat procedure impossibility, serious complications and mortality.</p></sec><sec><title>Aim</title><p>Aim: a search for imaging features providing prognostic data on the presence of certain mutations and gene expression in gliomas, obtained using traditional pulse sequences and characterized by the absence of restrictions on applicability depending on the tumors visible morphological features.</p></sec><sec><title>Material and methods</title><p>Material and methods: retrospective analysis of 49 eligible patients' primary brain MRI data between 2021 and 2023 from Almazov National Medical Research Centre (n = 31) and Napalkov Oncological Centre (n = 18) with glial tumors and subsequently identified status of the target variable; preprocessing of MR images using the histogram matching; regions of interest determination and semi-automated slice-by-slice segmentation with subsequent extraction of radiomics features; search for predictive radiomics features regarding the status of target variable using statistical analysis tools.</p></sec><sec><title>Results</title><p>Results. Dependence Entropy was found to be highly effective as a predictor of IDH mutations (area under the ROC-curve – 0.766 [0.627–0.880]).</p></sec><sec><title>Conclusions</title><p>Conclusions. We determined a target variable for the development of a predictive model (IDH status), a pulse sequence (T2-Tirm), a tool for initial imaging data preprocessing (histogram matching), regions of interest (tumor-associated T2-Tirm-hyperintensity including cystic and/or necrotic lesions). As a result, a statistically significant relationship between the Dependence Entropy feature and IDH status of glial tumors was found.
