Переломы проксимального отдела плечевой кости составляют до 75 % всех переломов плечевой кости у пациентов старше 60 лет. Количество таких пациентов постоянно растёт, однако нет единых взглядов на тактику лечения данной травмы.</p></sec><sec><title>Цель</title><p>Цель: определить эффективность применения лизата обогащённой тромбоцитами плазмы у пациентов с переломами проксимального отдела плечевой кости.</p></sec><sec><title>Объект и методы</title><p>Объект и методы. Проведено рандомизированное проспективное исследование, позволяющее оценить эффективность интраоперационного применения обогащённой тромбоцитами плазмы при остеосинтезе перелома проксимального отдела плечевой кости. Все оперируемые пациенты (n = 34) были распределены на три группы. В первую группу (основная 1) вошли пациенты, которым в конце оперативного вмешательства вводили лизат обогащённой тромбоцитами плазмы – 13 человек; во вторую (основная 2) – пациенты, у которых использовали богатую тромбоцитами плазму на 14 день от момента оперативного вмешательства, в третью (группа сравнения) – вошли пациенты, у которых лизат обогащённой тромбоцитами плазмы не применяли. Для объективизации влияния обогащённой тромбоцитами плазмы применяли ультразвуковой метод исследования.</p></sec><sec><title>Результаты</title><p>Результаты. При межгрупповом сравнении в разные сроки от операции показано, что в основной группе 1, которым богатую тромбоцитами плазму начинали интраоперационно, относительная толщина надостной мышцы на стороне операции была значимо ниже, чем в других группах. При сопоставлении результатов в группе сравнения и в основной группе 2 (которым лизат обогащённой тромбоцитами плазмы вводили на 14 сутки) относительная толщина надостной мышцы на стороне операции через 1 месяц и через 1,5–2,5 месяца от операции значимо не отличалась.</p></sec><sec><title>Выводы</title><p>Выводы. Интраоперационное применение обогащённой тромбоцитами плазмы позволяет снизить воспалительные изменения в сухожилиях надостной и подлопаточной мышц, что достоверно подтверждено данными ультразвукового исследования. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Proximal humerus fractures account for up to 75 % of all humerus fractures in patients over 60 years of age. The number of such patients is constantly growing, but there is no consensus on the treatment of this injury.</p></sec><sec><title>Target</title><p>Target. To determine the effectiveness of using platelet-rich lysate in patients with proximal humerus fractures.</p></sec><sec><title>Object and methods</title><p>Object and methods. A randomized prospective study was conducted. All operated patients (n = 34) were divided into 3 groups. The first group (main 1) included patients who were administered platelet-rich plasma lysate at the end of surgery – 13 patients. The second (main 2) included patients who used platelet-rich plasma on the 14th day from the moment of surgery. The third (comparison group) included patients who didn`t use platelet-rich plasma lysate. To objectify the effect of platelet-rich plasma used the ultrasound method of research.</p></sec><sec><title>Results</title><p>Results. An intergroup comparison at different times after surgery showed that in the Main 1 group, in which PRP was started intraoperatively, the relative thickness of the supraspinatus muscle on the side of the operation was significantly lower than in other groups. In a pairwise comparison in the group of patients without PRP and in patients of the 2nd main group (who were administered PRP n the 14th day), the relative thickness of the supraspinatus muscle on the side of surgery after 1 month and after 1,5–2,5 months didn`t differ significantly from surgery.</p></sec><sec><title>Conclusions</title><p>Conclusions.
