Артериальная гипертензия у реципиентов почечного трансплантата – основной фактор риска сердечнососудистых заболеваний, утраты трансплантата и преждевременной смерти. В посттрансплантационном периоде в 80 % случаев развивается стойкая или рефрактерная артериальная гипертензия, трудно коррегируемая привычной медикаментозной терапией. Трудности лечения заключаются в неэффективности многих препаратов первой линии, а также в том, что наиболее распространённые иммуносупрессивные препараты (циклоспорин, такролимус и метилпреднизолон) способствуют развитию гипертонии. Данный вид артериальной гипертензии представляет собой значительную проблему в клинической практике из-за сложности лечения и высокой летальности. Необходимость изучения терапии посттрансплантационной гипертензии обусловлена не только клинической значимостью, но и потенциальной возможностью улучшения результатов лечения и продолжительности жизни реципиентов почечного трансплантата.</p></sec><sec><title>Цель</title><p>Цель: изучение генеза, факторов риска, патофизиологии, диагностики и лечения посттрансплантационной артериальной гипертензии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: проведён анализ отечественной и зарубежной литературы с целью изучения посттрансплантационной артериальной гипертензии. Анализировано 37 литературных источников.</p></sec><sec><title>Выводы</title><p> Выводы. Высокое аретериальное давление подвергает реципиента почечного трансплантата риску сердечно-сосудистых заболеваний и смертности, а также усилению системной гипертензии, которая является как причиной, так и следствием почечной патологии. Артериальная гипертензия – модифицируемый фактор риска, способствующий прогрессированию почечной недостаточности. Не существует единого алгоритма лечения. Для достижения целевого артериального давления нередко необходимо применение нескольких ангтигипертензивных препаратов.  </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance: Arterial hypertension (HTN) in kidney transplant recipients is a major risk factor for cardiovascular diseases, graft rejection and premature death. In the post-transplant period in 80% of cases persistent or refractory arterial hypertension develops which is difficult to correct with conventional drug therapy. Treatment difficulties include the ineffectiveness of many first-line drugs and the fact that the most common immunosuppressive drugs (cyclosporine, tacrolimus and methylprednisolone) contribute to the development of hypertension. This type of hypertension represents a significant problem in clinical practice due to the complexity of treatment and high mortality. The need to study the treatment of post-transplant hypertension is due not only to its clinical significance, but also to the potential opportunity to improve treatment results and life expectancy of kidney transplant recipients.</p></sec><sec><title>Objective</title><p>Objective: to study the genesis, risk factors, pathophysiology, diagnosis and treatment of posttransplant hypertension.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: 37 literary sources were analyzed.</p></sec><sec><title>Conclusions</title><p>Conclusions: High blood pressure exposes the recipient of a kidney transplant to the risk of CVD and mortality as well as increased systemic hypertension which can be a cause and a consequence of renal pathology. Hypertension is a modifiable risk factor contributing to the progression of renal failure. There is no any single treatment algorithm.
